We report the case of a 39-year-old male patient who presented with a history of fever, and subsequent dengue serology confirmed the diagnosis. Upon arrival at our emergency department, the patient exhibited an altered sensorium, prompting a thorough examination. The Glasgow Coma Scale (GCS) revealed a score of E1M4V1.
Further investigation through a computed tomography (CT) scan unveiled a critical condition: the presence of a Right Frontotemporoparietal Acute Subdural Hematoma with Mass effect and midline shift exceeding 1cm. Immediate intervention was deemed necessary, but a complicating factor emerged as the patient’s platelet count was only 30,000.
Despite this challenge, the medical team at Chauhan Medicity Hospital acted promptly, ensuring the patient’s swift transfer to the operating room under platelet cover. The surgical procedure performed was a Right Frontotemporoparietal (FTP) decompressive Craniectomy with evacuation of the hematoma. Notably, the bone flap resulting from the procedure was strategically placed in the patient’s abdomen due to the severity of brain bulge.
Post-surgery, the patient showed a remarkable recovery and was discharged in a stable condition with no discernible neurological deficits.
