This position offers a rewarding opportunity to provide one-on-one care in a patient’s home environment—making a real difference where it matters most. Key Responsibilities

• Provide overnight personal care assistance
• Support patients with comfort, safety, and mobility needs
• Monitor and record vital signs when needed
• Assist with basic medical documentation and communication
• Collaborate with patients, families, and our nursing team to ensure high-quality care

Preferred Skills & Experience

• Experience in home care, nursing home, or hospital settings preferred
• Knowledge of dementia care or willingness to learn
• Reliable and compassionate with excellent communication skills
• Basic phone and computer literacy
• Open to on-the-job training for those new to home care
