Summary

Were currently seeking a reliable, skilled PRN Registered Nurse (RN) to join our home health CAPABLE (Community Aging in Place-Advancing Better Living for Elders) team. CAPABLE is a home-based, person-centered care model designed to help older adults live safely and independently in their homes for as long as possible.

The RN's Role in CAPABLE:

• The RN will work with CAPABLE participants to understand their goals, assess and suggest strategies in peoples homes who have volunteered to participate in CAPABLE.
• The CAPABLE program consists of up to four RN home visits over 4-5 months. Clinicians meet with participants in the persons home unless otherwise determined.
• The RN uses training and motivational interviewing techniques to conduct a comprehensive interview to identify and prioritize issues that matter to the participant. These goals become the foundation for brainstorming and action plan development and implementation.
• The RN documents the participants own goals in up to 3 Action Plans.

The RN collaborates closely with the occupational therapist and handyman to ensure that the home environment and care strategies support the individuals goals and promote aging in place.

The CAPABLE nurses duties are performed remotely from home and will require intermittent travel to conduct in-person home visits.
