Tender Care Home Health & Hospice currently has excellent career opportunities for a Certified Nursing Aide (CNA) to provide individualized, goal-directed care

Job Responsibilities:

• Follows the patient's plan of care as prepared by the Registered Nurse.
• Performs simple procedures as an extension of nursing and therapy services. 
• Communicates effectively with patient and/or family in all aspects of care being provided.
• Responds quickly and appropriately to customer needs.
• Demonstrates interpersonal skills of respect, sensitivity, cooperation, openness, flexibility, positive attitude, and pleasant manner.
• Follows universal precautions.
• Gives personal care to patients; including, but not limited to, bathing, shampoo, shaving, mouth care and skin care.
• Understands the basic elements of body functioning and communicates any changes in the patient's condition or incidents to immediate supervisor or other appropriate member of the Interdisciplinary team.
• Performs household services essential to health care at home as per the plan of care.
• Provides an environment, which promotes respect for patient, privacy and property, and creates a successful interpersonal relationship with the patient and family.
• Immediately reports to the Registered Nurses/Nursing Supervisor changes in the patient's condition and needs.
• Completes appropriate documentation according to Tender Care Home Health policies.
• Attends all mandatory in-service programs and staff meetings.
• The Home Health Aide is primarily responsible to the Registered Nurse in charge of their patient assignment.
• Demonstrates commitment, professional growth and competency by completing 12 hours of education per year.
