Senior LTSS Assessor (RN) – Field-Based
Location: Steuben County, NY (covering Steuben, Chemung, Schuyler, and Yates counties)
Position Type: Full-Time, Field-Based
Pay Range: $35.49 – $63.79/hour + 8% Annual Bonus Incentive Option

Position Overview:
As a Senior LTSS Assessor, you will perform field-based assessments for long-term care members, conducting 2–4 UAS Assessments or Reassessments daily. You will evaluate service needs, develop care plans, and coordinate medical and supportive services for high-acuity members. This role requires travel within Steuben and surrounding counties and direct interaction with members, families, and care teams.

Must-Haves:

· Current NY Licensure as a Registered Nurse (RN)

· Residency in Steuben County to cover Steuben, Chemung, Schuyler, and Yates counties

· Ability to conduct home and/or other site visits M–F

· 2–4 UAS Assessments or Reassessments per day

· Driver’s License (personal transportation preferred; public transportation considered)

· Minimum 4 years of relevant RN experience

· UAS Assessment experience

Required Skills:

· Strong nursing skills; Bachelor’s degree in Nursing preferred

· Experience with UAS assessments and care coordination

· Strong communication and education skills for interacting with members, families, and caregivers

· Ability to develop and manage care plans and collaborate with care management teams

Preferred Skills:

· Experience in care management, preferably in a Managed Care Organization (MCO)

· Knowledge of community resources and coordination of services

· Leadership skills to precept clinical new hires and improve quality of care

Key Responsibilities:

· Evaluate complex/high-acuity members to develop personalized care plans

· Coordinate and manage care with members, families, and care providers

· Monitor member progress, conditions, and care plan effectiveness

· Facilitate member education on disease management, services, and benefits

· Serve as liaison and advocate between members, families, providers, and facilities

· Conduct home and site visits as required

· Precept new clinical hires and support onboarding and competency development

· Ensure compliance with state and federal regulations and clinical guidelines

· Collaborate with leadership to improve quality of care and service delivery

Education & Experience:

· Graduate from an accredited School of Nursing or a Bachelor’s degree required

· Minimum 4–6 years of related RN experience , preferably in care management

· Experience working with complex/high-acuity populations preferred

Licensure/Certification:

· RN – Registered Nurse (NY State Licensure required)
